Abstract

A fall onto an outstretched hand (FOOSH) is a common cause of wrist injuries, particularly in elderly individuals with brittle bones. Such injuries may result in instability of the distal radioulnar joint (DRUJ), which plays a crucial role in forearm rotation. Restoring joint stability after injury is essential to heal wrist ligaments and prevent long-term complications such as abnormal motion or functional loss of the wrist. The suture button fixation technique, originally developed for ankle syndesmosis repair, has gained increasing attention for wrist stabilization. This study aimed to evaluate the effectiveness of three suture configurations, namely single transverse, single oblique, and a combination of transverse and oblique, using Finite Element Analysis (FEA) to simulate the biomechanical behavior of the DRUJ under various loading conditions. The objective was to compare the stability measured by DRUJ distance achieved by each technique systematically and quantitatively. The results showed that a combination of transverse and oblique technique provided the smallest value of DRUJ distance across all directions. It demonstrates superior effectiveness in resisting joint translation under rotational, tensile, and compressive loads.
