การเปรียบเทียบผลการรักษาของการทำศัลยกรรมปริทันต์เพิ่มความยาวฟันเพื่อการบูรณะด้วยเลเซอร์ Er,Cr:YSGG โดยวิธีไม่เปิดแผ่นเหงือกกับวิธีเปิดแผ่นเหงือก

Abstract

The present study aims to compare the periodontal outcomes of Er,Cr:YSGG laser flapless crown lengthening procedure and conventional technique. Methods: Twenty-five participants were divided into two groups: 13 patients were treated with crown lengthening procedures by open-flap and bone recontouring and 12 patients were treated with crown-lengthening procedures using a flapless Er,Cr:YSGG laser. Their periodontal status was measured and compared at baseline, immediately, one month, and three months after surgery. Results: The results showed a significant increase in clinical crown length immediately after surgery in both groups. After a three-month follow-up, the gingival margin of laser group remained at stable height with 0.17 ± 0.31mm increase after surgery, while the gingival margin of the conventional group showed both recession and rebounding by a -0.13 ± 0.63 mm (p>0.05) average. Conclusions: Both techniques for crown lengthening procedure provided predictable outcomes. However, the flapless Er,Cr:YSGG laser crown lengthening was more easily performed, less time-consuming, minimally invasive and without flap reflection and less bleeding, and no need for suturing and periodontal dressing. This technique may be an alternative treatment for providing enough height of tooth for restoration.

วัตถุประสงค์เพื่อศึกษาการเปลี่ยนแปลงสภาวะปริทันต์หลังได้รับการทำศัลยกรรมปริทันต์เพิ่มความยาวฟันโดยวิธีไม่เปิดแผ่นเหงือกด้วยเลเซอร์ Er,Cr:YSGG เปรียบเทียบกับวิธีเปิดแผ่นเหงือกโดยใช้มีดผ่าตัดร่วมกับการตัดแต่งกระดูกด้วยเครื่องกรอ โดยมีอาสาสมัคร 25 คน แบ่งเป็น 2 กลุ่ม คือกลุ่มที่ได้รับการทำศัลยกรรมปริทันต์เพิ่มความยาวฟันด้วยวิธีเปิดแผ่นเหงือกร่วมกับการตัดแต่งกระดูกด้วยเครื่องกรอจำนวน 13 คน และกลุ่มที่ได้รับการทำศัลยกรรมด้วยเลเซอร์ Er,Cr:YSGG โดยไม่เปิดแผ่นเหงือกจำนวน 12 คน ทั้ง 2 กลุ่มได้รับการตรวจสภาวะปริทันต์ที่เวลาก่อนทำศัลยกรรม หลังทำศัลยกรรมทันที 1 เดือน และ 3 เดือน ผลการศึกษาพบว่าฟันที่ได้รับการทำศัลยกรรมทั้งสองกลุ่มมีความยาวตัวฟันเพิ่มขึ้นหลังการทำศัลยกรรมทันทีอย่างมีนัยสำคัญ ในกลุ่มเลเซอร์ที่เวลา 3 เดือน มีขอบเหงือกมีระดับคงที่หรือเปลี่ยนแปลงเพิ่มขึ้นจากหลังทำศัลยกรรมทันทีเพียงเล็กน้อยโดยมีค่าเฉลี่ย 0.17±0.31 มิลลิเมตร ส่วนกลุ่มที่เปิดแผ่นเหงือกพบทั้งการร่นเพิ่มและการกลับขึ้นไปของเหงือกไปทางตัวฟัน แต่มีค่าเฉลี่ย -0.13±0.63 มิลลิเมตร (p>0.05) สรุปผลว่าการทำศัลยกรรมปริทันต์เพิ่มความยาวฟันด้วยเลเซอร์ Er,Cr:YSGG โดยวิธีไม่เปิดแผ่นเหงือกสามารถเพิ่มความยาวของตัวฟันได้ตามวัตถุประสงค์ไม่ต่างกับวิธีเปิดแผ่นเหงือกตามปกติ แต่ทำได้ง่าย ใช้เวลาน้อยกว่า มีเลือดออกน้อย ไม่ต้องเย็บแผล และไม่ต้องใช้วัสดุปิดแผลปริทันต์ ถือเป็นทางเลือกที่ดีในการทำศัลยกรรมปริทันต์เพิ่มความยาวฟันเพื่อการบูรณะ
